Forms

It is the student's responsibility to be sure all required Graduate School forms are filed with the Graduate Division in MS 207.  The forms will be processed and forwarded to the appropriate office(s) for final approval if necessary.  It is imperative that the timelines for all forms are adhered to.  This could jeopardize your progress toward your degree and/or ultimately your graduation.

All students shall be assigned an advisory committee in the student's program or department no later than one year after admission to the program or laboratory. Once you have selected your permanent lab, you should consult with your mentor to form an Advisory Committee which should consist of at least two members from your major area and one from another area. You should work with the Graduate Assistant in your department that handles student records so that your student file is always current. This form, as should all of your graduate school forms, is to be turned into the IUSM Graduate Division (MS 207) after you have all the faculty signatures for the dean's approval.

This committee consists of a professor who will agree to direct the student's dissertation. The department shall then recommend to the dean for approval a research committee composed of the chosen director (who will normally serve as chair of the committee), two or more additional faculty members from the major department, and a representative of each minor. The student will submit with the research committee form a dissertation prospectus of the dissertation research and if the research involves human subjects, animals, biohazards or radiation, approval from the appropriate university committee must be obtained. The membership of the research committee and the dissertation prospectus must be approved by the University Graduate School at least six months before the defense of the dissertation.

Following the passing of the qualifying examination and the completion of all course work and departmental language or research-skill requirements (if any), the student's advisory committee will submit a Nomination to Candidacy form to IUSM Graduate Division (MS 207). The qualifying examination must be passed and submitted at least eight months before the date the degree is awarded. Upon approval of the dean, the student will be admitted to candidacy. Once this form has been filed, the date of the successful completion of the qualifying examinations is the one used in determining the seven-year periods (ten years for MD/PhD) for currency of courses and completion of the dissertation. The student must have continuous enrollment once they have been admitted to candidacy.

As required by the School of Medicine Graduate Division, the qualifying exam form is used to record the result of a qualifying exam (i.e. the exam that PhD program used to determine transition of a doctoral path student to candidacy). If the program exam format involves several steps, this form should be used to report the final deciding part of the exam. Your mentor should complete and submit this form to the IUSM Graduate Division (MS 207). A copy should be retained for the program student record and the student should receive a copy of this form. The qualifying examination must be passed at least eight months before the date the degree is awarded and submitted.

IBMG Students are responsible for completing the student name and student University ID section of this form. The student is also responsible for collecting all necessary signatures and returning the form to the IUSM Graduate Division (MS 207). It is recommended that the student starts communicating with all those involved at least two weeks prior to the deadline.

Additional Forms Not Available Directly from the IUSM Graduate Division

 

Application for Advanced Degree

Students seeking a master's degree must submit an application for advanced degree at least 60 days prior to the 10th of the month the student intends to graduate.  This form, as should all of your graduate school forms, is to be turned into the IUSM Graduate Division (MS 207) after you have all the faculty signatures for the dean's approval. 

This form is available from the IUPUI Graduate Office.

 

Recommendation for Advanced Degree

Students seeking a master's degree must submit the recommendation for advanced degree at least 30 days prior to the 10th of the month the student intends to graduate.

The requirements for all master's degrees must be completed within five consecutive years.  Masters students do not have to maintain continuous enrollment but must be enrolled in the semester they will be graduating.
